We understand you have a busy schedule, so when preparing to visit the dentist, please remember that your appointments can last between 45 and 90 minutes.
NEW PATIENTS
First Visit
Please remember to bring these items with you to your first appointment:
- Completed New Patient Forms (you can fill them out in advance using the downloadable links above, or complete them at check in)
- Your insurance card
- Your picture ID
You should also bring any past dental records and / or x-rays, any pertinent medical records, and a list of any medications you are taking (including prescriptions, over the counter medications, and herbal supplements or vitamins.)
It is very important that you let us know if you are on heart medication, aspirin, or anticoagulants, or if you have any conditions such as diabetes, high blood pressure, artificial heart valves or joints, a history of rheumatic fever, or any other serious medical condition.
